We are excited to offer you the GREAT SMILE PREMIER PLAN, a dental discount plan!

Benefits of our dental plan include:

No annual maximum. No deductible. No wondering whether insurance will pay toward your treatment. No waiting period, which means you can start treatment TODAY! No preauthorization is ever required, and cosmetic dentistry is included!

Membership benefits:

• Initial comprehensive exam, X-rays, and dental cleaning (Prophylaxis or Periodontal Maintenance)

• Note: Scaling and Root Planing or "Deep Cleanings" due to periodontal disease is considered as a treatment. 

• Six-month checkup with exam and dental cleaning (Prophylaxis or Periodontal Maintenance. An additional PM may be added for patients with periodontal disease based on Doctor's discretion.)
• All dental treatment completed within 12 months of enrollment will be discounted by 15% if paid in full at time of service.
• This includes elective cosmetic procedures like teeth whitening, crowns, and veneers (products are discounted 5%)
• Annual cost for individual plan: $500.00

• Annual cost for spouse or adult dependents age 15 and up (living in the some household): $420.00
• Annual cost for children under the age of 4 (living in the same household) $175.00
• Annual cost for children ages 5-14 (living in the same household) $250.00

• Annual cost for individual child under age of 4 Plan: $180.00

• Annual cost for individual child between ages of 5-14 Plan: $300.00

The Fine Print - There's no ID card, no group or member number to bring! All your membership information will be kept in your electronic record. Your effective date is the day you sign up an dyour renewal date ist he same date every year. This is a dental discount plan and is NOT dental insurance. It CANNOT be combined with any other dental insurance or combined with any other offers. It is good ONLY at the office of Dr. Dick Yang and Dr. Keira Yang/ Great Smile Dental. Therefore, if you are referred to a specialist, they will NOT offer this discount. Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medcal care, disability or workman's comp type of insurances are involved, this discounted plan CANNOT be used. This is a non-refundable plan. No refunds if the patient chooses not to utilize their plan Plan is subject to change yearly. All payments are due AT TIME OF SERVICE to receive the discount. Plan fees are valid ONLY when paid at time of enrollment. Any services received that are not paid at the time of service, will be billed at the usual higher fee.
